“Providence Row is delighted to be in partnership with the Charles Plater Trust in support of our Routes to Roots (R2R) project in Tower Hamlets. This partnership enables us to deliver vital advocacy support to 75 extremely vulnerable homeless people, annually, as they approach discharge following hospital treatment. Our R2R case workers help patients access accommodation, benefits and ongoing health and community-based support in their new neighbourhoods. Without our support, these clients risk returning to the streets following treatment with a high risk of falling ill again and returning to A&E for the same condition”.
